72 year old male presented with unstable angina; history of moderate Aortic Stenosis (AVA 1.4 cm2). Coronary angiography demonstrated significant calcified distal Left Main stenosis and stenosis of the origins of the LAD and Circumflex; A spiral dissection of the Right Coronary Artery developed on injection of the RCA that was stented with Bare Metal stents; the patient experience ongoing pain – an intra-aortic balloon pump was inserted and the patient was sent for urgent coronary artery bypass surgery and Aortic Valve Replacement
